I thought the characters already knew that Adrian and Miriam are half-siblings.  I'm lost.

My interpretation of this is that Jacqui is 100% certain that Patrick is Miriam's father, but is less certain that he is Adrian's father.  This is the first time that the possibility of Adrian not being the child of Patrick and Miel has been raised in-comic.
